feat(typeorm): add sqlcommenter-typeorm package
Adds @query-doctor/sqlcommenter-typeorm with dual CJS/ESM build support. Patches QueryRunner.query() via DataSource.createQueryRunner() interception to append sqlcommenter tags (db_driver, file, route, W3C trace context) to all TypeORM queries. Uses alreadyHasTrailingComment() to coexist with TypeORM's QueryBuilder.comment() which prepends comments. Includes integration tests using EntitySchema + sql.js exercising Repository.find(), .save(), .findOneBy(), and QueryBuilder pipelines.
