Switch to Esplora's `get_block_hash()`/`get_header_by_hash()`

We should switch the ChainAccess to use get_block_hash() after the upcoming release of rust-esplora-client.

Besides for retrieving the necessary data for the Confirm methods, it should also be used for Access (see #21).
