Prepare OpenCode Goal 1.3.28 after merging the long-running steering and plan reliability fixes.

• Bump package version to 1.3.28.
• Advance the one-shot trusted npm publish workflow to 1.3.28 with 1.3.27 as the required authoritative predecessor.
• Add changelog and dedicated release notes covering auto-stall steering recovery, adaptive Todo-aware stall windows, stale Todo revision binding, and unlimited-by-default turn budgets for new Goals.
• Preserve explicit finite budget guards and explicit user pause semantics.

The feature head passed CI, real host progress, restart recovery, Loop 0.5.35 coexistence, and all functional Release Readiness package-smoke steps before merge.
