fix(edgeone): inject custom HTML into SPA shell

Summary / 摘要

• 修复 Issue 全局设置中的‘自定义内容’和‘自定义头部’配置后不生效 #49：EdgeOne 的 HTML 导航不再直接落到静态 index.html，而是通过 middleware 透明改写到专用动态 SPA 路由，由 Node 云函数返回页面壳。

• 从已成功加载的数据库快照注入 customize_head / customize_body；存储不可用时返回原始 SPA 壳，不因默认空库丢失占位符。

• 重建 HTML 时保留 CSP 等安全响应头，移除失效的 ETag、Content-Encoding、Content-Length、Last-Modified，并保持 Cache-Control: no-cache, must-revalidate。

• DB trust 改为绑定具体快照，避免并发请求间读取模块全局信任状态产生竞态。

• 清理自定义内容中的保留锚点，避免最新版前端再次运行时注入。

• 保留 EdgeOne 官方 /* -> /index.html SPA fallback 语义：静态资源和 functions 先匹配，未命中才回退；/api、/d、/p、/sd、/dav、/s3、/kv-*、/health 不改写。

• 同步重建 cloud-functions/[[default]].js。
