The config server accepts "adhoc": true for keys not declared in any org schema (its 400 error prescribes it), but th config set never sent it — release-tooling secrets had no th-config path. --adhoc requires an explicit --tier (clap-enforced) so the write states its sensitivity out loud (ADR-075 spirit).

Verification: 486 smooth-cli tests green; fmt clean; used live to store the Big Smooth TestFlight secrets (matchPassword, ascKeyId/IssuerId/KeyP8) in the Smoo AI prod org.

The server has always accepted adhoc:true for keys declared in no org
schema (its 400 message prescribes exactly that), but the CLI never sent
it — so release-tooling secrets (match passphrase, ASC key material) had
no th config path without inventing a consumer schema. --adhoc requires
an explicit --tier (clap 'requires'), keeping ADR-075's say-it-out-loud
property for sensitivity. Second cmd_set caller (limit set) passes false.

First use: Big Smooth TestFlight signing secrets → Smoo AI prod org.
