Error messages only carried an HTTP status, matching neither this provider's own convention elsewhere nor OpenClaude's aimlapi client (which always labels a failure with the request's origin/path) — made a reported "HTTP 404" impossible to attribute to a specific step without asking the reporter to re-run under a debugger.

Error messages only carried an HTTP status (
AI/ML API request failed (HTTP 404)), which made a reported failure impossible to attribute to a specific step. Now includes the method + URL, matching OpenClaude's own aimlapi client convention.
