What is the current behavior?
Language-server synchronization does not isolate clients by effective PostgreSQL context.
What is the new behavior?
Manages a PostgreSQL Language Server client pool per effective context, synchronizes lifecycle changes, and reports managed-client failures.

Depends on the PgLS session context RPC being available in the configured binary.
