Changed "Lifecycle of Publishers and Subscribers" to "Lifecycle of a Combine Subscription" as it is more appropriate here because we are not discussing when publishers or subscribers are created or destroyed. Rather, we are discussing when a subscription is created, setup, used, and completed.

Clarified the docs for the issue #262
And also modified a section title from "Lifecycle of Publishers and Subscribers" to "Lifecycle of a Combine Subscription".
And also fixed some grammar mistakes.
