docs: fix CI badge after the test.yaml split#122

The split removed test.yaml, breaking the single CI badge. Add a badge per workflow (breaking, changelog, diff, pr-comment) and shorten each workflow name so the badge labels and the Actions tab read cleanly. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>
