Implements the first stable low-memory CodebaseGraph slice: schema-v3 refresh and materialization settings, cross-process refresh leadership, bounded watcher admission and overflow recovery, refresh-only after-lock no-op publication, expanded health telemetry, storage lock tests, Scryer responsibility folding, and validated architecture wiki updates.\n\nValidation: cargo test --workspace --all-targets (the sandbox-blocked loopback preview test passed separately with loopback permission); focused refresh, MCP, watch, and storage suites pass.
