Create releases from existing tags

Summary

• omit target_commitish when creating a GitHub Release
• rely on the existing immutable release tag instead of asking the integration to target an older commit
• avoid requiring the GitHub App to have Workflows write permission

This fixes the repeated Resource not accessible by integration response during manual release recovery.
