Summary

A dynamic skill resolver failure at turn.started is logged and ignored, so model work can continue with stale instructions. This draft adds a focused regression for #3288 before changing runtime behavior. The test requires the existing recoverable boundary error and prevents partial publication from a concurrent successful resolver.

Closes #3288

Validation

pnpm --filter eve build:compiled
pnpm --filter eve exec vitest run --config vitest.unit.config.ts src/context/dynamic-skill-lifecycle.test.ts -t 'rejects a failed turn refresh' fails on origin/main as expected: the resolver logs threw — skipping, and the dispatch resolves instead of rejecting
