Recognize AI Gateway credit and budget refusals

[e0-gh-vercel-connect]

AI Gateway's HTTP 402 credit and project-budget refusals now produce stable semantic summaries instead of falling through to an unclassified failure.

What changed

• Expose Gateway error discriminators nested in an upstream response body to the semantic-error signal extractor.
• Add recoverable catalog rules for insufficient_funds and quota_for_entity_exceeded.
• Keep sentence matching as a fallback for error shapes that omit the discriminator.
• Preserve the focused regression cases from the red commit for both wrapped Gateway errors and message-only fallbacks.

This keeps the existing 402 recovery classification unchanged while allowing journal consumers to key on gateway-credit-exhausted or gateway-budget-exhausted across the model-call and terminal-session failure paths. See #3321.

Validation

• Focused semantic-error and model-call suites: 88 tests passed.
• TypeScript check: passed.
• oxlint on changed files: passed.
• oxfmt check on changed files: passed.
• Full unit command: 8,660 tests passed; 9 unrelated build-dependent or timeout failures remain because the checkout lacks generated eve/* package artifacts for self-modification tests and two tests exceeded their existing 5-second timeout.

[e0-gh-vercel-connect]

The failed run was a real regression on stale commit 4d4aba2: all four new Gateway credit/budget semantic-error cases returned undefined.

PR #3322 has since advanced to 5419dcb6, adding the missing nested Gateway error extraction and catalog rules. The focused suite passes locally: 48/48 tests.

The replacement CI run completed successfully, including unit, integration on Ubuntu and Windows, scenario, TUI, lint, and typecheck. I did not rerun the old failed workflow because it targeted superseded code.
